If you would rather fill them out in the office, or you are not sure whether you need to complete them, let us know when we confirm your appointment.
If you need help with the forms
If the online forms are difficult for you to use for any reason, please call or text (310) 305-4644. We can go through them with you over the phone, provide them in another format, or complete them together at your visit. There is no cost for this, and it will not delay your care. Our accessibility statement has more on the accommodations available, including sign language interpreters and real-time captioning.
Your privacy
What you enter goes directly to NexHealth under a business associate agreement and is handled as part of your dental record, protected under HIPAA. Our Notice of Privacy Practices describes how that information may be used and the rights you have over it.
