Appointments are reserved specifically for each patient. These policies help the office protect clinical time, coordinate care, and provide appropriate notice to everyone scheduled.
Appointment policy
We reserve appointment time especially for you. Longer procedures also involve preparation, staffing, and laboratory coordination, so we ask for at least 48 hours’ notice for any change. Adequate notice allows us to offer the time to another patient.
Standard appointments
Changes, cancellations, and no-shows with less than 48 hours’ notice may incur a $250 fee. Appointments rescheduled within the same week may incur a $200 fee.
Longer appointments
For appointments longer than two hours, changes may be subject to 50% of the scheduled procedure fee or the applicable deposit or fee on file.
If you need to reschedule
Please call or text (310) 305-4644 as early as possible so we can offer the time to another patient.
Privacy policy
This policy explains what information Linden Dental Care collects through this website, how that information is used, and the choices available to you. It covers the website. Health information created or received in the course of your care is governed separately by our Notice of Privacy Practices.
Information we collect
Through the consultation request form, we collect what you choose to provide: your first and last name, phone number, email address, preferred contact method, whether you are a new or existing patient, any preferred days or times, how you heard about us, and whether you would like a therapy dog present. The form is deliberately limited to scheduling details and does not ask about your symptoms, treatment, or medical history. Providing this information is voluntary. You are always welcome to call or text the office instead.
If you opt in to receive text messages, we collect your phone number and your consent to send them.
As on most websites, our hosting provider automatically records limited technical information such as IP address, browser type, and pages visited. This is used to keep the site running, secure, and working correctly.
How we use it
To respond to your request, to arrange or coordinate your care, to communicate with you through the method you selected, and to operate and improve this website. We do not sell your personal information, and we do not share it with third parties for their own marketing.
Service providers
Consultation requests are submitted through our website host, which receives each request, stores it, and passes it on to the office. Because the form asks only for scheduling details, no health information travels this way. Your new patient forms and your dental record are handled separately through NexHealth, under a business associate agreement that requires it to protect that information and use it only to provide that service to us. Typefaces on this site are served by Google Fonts, and the map on our Visit page is embedded from Google Maps, which means Google receives your device's network address when those load. These providers process information only as needed to deliver their service to us.
Please keep clinical detail off this site
The form and text messaging are intended for scheduling and general questions. Please do not include medical history, diagnoses, photographs of a clinical concern, or insurance identifiers. For anything of that nature, call the office at (310) 305-4644.
Text messages
What we collect. When you opt in to receive SMS messages, we collect your phone number and your consent to send SMS messages.
How we use it. To send you messages about scheduling and your care at this practice, appointment reminders, confirmations, and replies to questions you send us. We do not send marketing or promotional text messages.
Sharing. SMS opt-in and phone numbers collected for the purpose of SMS are not shared with any third parties or affiliate companies for marketing purposes.
How often. Message frequency varies and depends on your appointments.
Help. Reply HELP to any message, or call the office at (310) 305-4644, and we will help.
Opting out. You can opt out of receiving SMS messages at any time by replying STOP to any message we send you.
How long we keep it
We keep consultation requests only as long as we need them to respond to you and arrange your visit. If you become a patient, the information you gave us moves into your dental record, which is kept for the period required of dental practices in California and is governed by our Notice of Privacy Practices rather than this policy. You can ask us to delete a request at any time.
Reviewing or changing your information
To review, correct, or ask us to delete information you submitted through this website, call or text (310) 305-4644, or ask at the front desk. We will respond within a reasonable period. If the information is part of your dental record, the rights described in our Notice of Privacy Practices apply instead.
Do Not Track
Some browsers can send a Do Not Track signal. There is no common industry standard for how websites should respond to it. This website does not track visitors across other websites for advertising, and we do not alter our practices in response to a Do Not Track signal.
Children
This website is intended for adults. We do not knowingly collect personal information online from children under 13. A parent or guardian should submit any request on behalf of a patient under 18.
Data protection
We implement reasonable measures to protect your personal information from unauthorized access or disclosure. No method of transmission over the internet is completely secure, which is why we ask you to keep clinical detail off this site.
Accessibility
If any part of this website or our office is difficult for you to use, please read our accessibility statement or call the office so we can help.
Changes to this policy
If we make a material change, we will revise the date at the foot of this page and post the updated policy here.
Terms of use
